WAIVER AND RELEASE OF LIABILITY FORM

In consideration of being permitted to participate in the activities of PRIBL Baseball, LLC d/b/a Puerto Rico Independent Baseball League (referred to collectively as the PRIBL) in any capacity, including but not limited to, playing in league games or practices, tryouts, coaching, or being on the field, in a dugout, or a spectator area for any purpose whatsoever, and fully understanding that participation in the game of baseball includes the risk of serious personal injury, I do fully and assume all risk of injury to myself, including, but not limited to fatality, and do hereby unconditionally and release, even for their negligence, indemnify and hold harmless the following: Carlos Nuñez; PRIBL Baseball, LLC d/b/a Puerto Rico Independent Baseball League; any other members, officers, volunteers, game officials, or persons affiliated with PRIBL Baseball; all landowners of facilities that may sanction, or give permission for use of facility for play or practice; any governmental agencies involved with PRIBL Baseball or the facilities at which games or practices may take place; any organizations involved in the sponsorship of teams competing within PRIBL Baseball; from all claims for damage or injuries of any kind, now or in the future.

Furthermore, I am physically fit to participate in the daily program activities. I hereby authorize the staff of PRIBL Baseball to act for me in an emergency.

I agree to abide by and obey any League Rules set forth by PRIBL Baseball. I fully agree to accept any suspensions or disciplinary actions that PRIBL officials shall hand down according to any behavior on my part that may, at PRIBL’s sole discretion, warrant such action by PRIBL.

I further grant PRIBL Baseball and/or its agents the use of my likeness in any photographs, video recordings, motion pictures, or other records of the event for any reasonable purpose without compensation.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND HAVE AGREED TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY AGREEMENT TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

REFUND POLICY

Your registration is a firm commitment to participate in one of the PRIBL Baseball camps. By registering, the submitting player and/or third-party paying for the player agree that no refunds will be issued for any reason. All fees paid are to cover in advance the stay of the player in the camp. This registration and payment guarantee one player roster spot within one of the PRIBL Baseball camps. PRIBL Baseball will not grant refunds of any kind, regardless of the player’s feelings about their performance, the third-party instructors’ evaluations, or the player’s opinion of PRIBL. This payment is specifically for the opportunity to participate in one of our camps. It does not guarantee or represent the player a contract with any team or league after the camp. If a player cannot attend, he will receive credit for the following year.

This payment is for the opportunity to participate as a player in the 2023 Puerto Rico Independent Baseball League. By paying, the submitting player and/or third party paying for the player agrees that NO REFUNDS will be issued for any reason. A credit for a future camp will be issued if the player cannot attend the event.

The Player Participation Fee doesn't include Airfare, and the payment consists of housing, meals, and transportation during the camp.
